Placement Application Form for London School

Found a work experience placement? Before filling in the application form, gather the information listed below.Once submitted, London School will review your form and let you know if anything needs changing before it can be processed and approved.

Before you start

You will need the following information to complete the form:

•Employer's name
•Address
•What the employer does

Work placement details
•Job title
•Description of work placement activities
•Days and hours of attendance each week
•Dress code

Insurance detailsYou will need to know whether the employer has Employer's Liability Insurance. If so, you will need:
•Insurance company name
•Insurance number
•Insurance expiry date
4.
Employer and Parent/guardian contactThe name and email address of a contact at the employer and parent or guardian who has approved the placement. A digital copy of the form and all application updates will be sent to this person.
